The problem
Clinicianswerecarryingpaperbecausetheexistingwebsystemwasunusableonaphoneandunreachableinpartsofthebuilding.Noteswerewrittentwice:onceonpaperatthebedside,onceintoaterminalhourslater.
The double entry created a lag between what happened to a patient and what the record said had happened, a clinical risk as much as an efficiency problem.
What we did
We designed for the ward round, not the desk: one-handed operation, large targets, and a flow that assumes interruption. Everything is written locally first and synced when the device finds a network, with conflict resolution that never silently discards a clinician's note.
Dictated notes are structured into the record's schema with the practitioner confirming before anything is saved. Sensitive data never leaves the health network's Azure tenancy.
We ran fortnightly sessions with clinicians on real devices for the length of the build. Roughly a third of our initial assumptions did not survive contact with an actual ward.
